The Lewis Jewel must be ordered by filling out this application form. Eligibility must be verified by the Grand Lodge Office.

The Lewis Jewel is permitted to be worn on Masonic occasions by Master Masons whose fathers are Master Masons in good standing (or were at the time of their death) and likewise by Master Masons whose sons are Master Masons in good standing (or were at the time of their death).
